Great Boca Smiles Membership Plans
Great Boca Smiles is proud to offer two annual membership plans:
- No deductible
- No pre-authorizations
- No waiting periods
- Discounted fees on cosmetic dentistry
Terms and Limitations of the Plan:
There’s no ID card, no group number, or member number to bring! All of your membership information will be kept in your electronic record. Your effective date is the day you sign up and your renewal date will be one year on the 1st or 15th of the month you join. The plan covers dental services only, not products.
Program Disclaimer:
All plan fees are due at the time of Enrollment. An additional fee ($150.00) will be charged for any missed, canceled or broken appointment without 24 hours prior notice This plan is non-transferable/non-refundable. This plan cannot be combined with any other insurance, dental savings plan, coupon, or discount. Plan participants are responsible for scheduling their periodic treatments. Services not utilized at the end of each year’s membership period are not carried over to the following year. You are required to renew on your renewal date. A $50.00 re-enrollment fee in addition to the yearly membership will be incurred if there’s a lapse. Discounts apply only to services rendered by Great Boca Smiles, it does not include treatment for services provided elsewhere even if you are referred to a specialist by Great Boca Smiles. In addition, CareCredit cannot be used by patients enrolled in the membership plan.
Without dental coverage, most people are discouraged from obtaining proper diagnostic, preventive, and restorative dental care. Our dental membership provides patients with an affordable dental plan that covers all of their essentials.

- 2 Preventive dental cleanings a year or equivalent valued towards Full Mouth Debridement Gingival Scaling
- 2 Doctor Exams
- Needed X-rays.

(Perio savings of $270.00)
Membership Includes:
- 3/4 Perio Maintenance cleanings a membership year
- 2 Doctor Exams
